How do I upload documents?
There are multiple areas within Customer Connect that allow users to upload documents directly to CARF.
- Application - Application Items page (Note: application items are not always required)
- Survey - Upload Quality Improvement Plan accreditation surveys
- Survey - Upload Plan of Action certification surveys
- Resources - Ongoing Communication Forms
The following restrictions apply to documents being uploaded to Customer Connect:
- File size is limited to 10 MB per document
- Allowed file types: .doc, .docx, .pdf, .xls, .xlsx, .rtf, .txt, .ppt, .pptx, .gif, .jpg, .bmp, .tif, .tff, .tiff, and .zip
- File name must not contain pound (#), apostrophe ( ' ), or multiple periods (for example: Budget_03.18.xls)
